Dear Parent/Guardian/ player, Please read the following carefully and provide your consent for all statements together by checking the box. Players over 18 can give consent for themselves, and players under 18 (minors) will need parent/guardian consent.
1. Participation Consent
I hereby consent to my/ my child’s participation in training sessions and related activities conducted by Sports For Life (SFL) and its partner academies. I agree to ensure compliance with all applicable rules, guidelines, and policies.
2. Acknowledgement of Risk and Waiver of Liability
I understand that participation in sports involves inherent risks, including but not limited to physical injuries such as sprains, fractures, concussions, or injuries to bones, ligaments, muscles, or internal organs, all of which can also be fatal or cause disability. I voluntarily assume all such risks and agree that Sports For Life (SFL), its academies, partners, affiliates, and their respective staff shall not be held liable for any injury, death, permanent disability, loss, damage, or expense arising from my/my child’s participation, except in cases of gross negligence or wilful misconduct.
3. Safety Measures and Emergency Medical Consent
I acknowledge that SFL and its partners will take all reasonable precautions to ensure safety and provide immediate first aid. However, I understand that external medical or hospital intervention may be required in certain situations. I agree that reasonable efforts will be made to contact me in case of an emergency for my child (parent/guardian for minor players)/ my kin (for players over 18). If I (parent/guardian for minors)/my kin (for players over 18) are unavailable, I authorise SFL and its representatives to arrange for and consent to necessary medical treatment, including emergency/life-saving procedures, as deemed appropriate by qualified medical professionals.
4. Medical Expenses
I agree to bear full financial responsibility for any medical expenses incurred as a result of my/my child’s participation, including costs not covered by insurance.
5. Rules, Conduct, and Responsibility
I confirm that I have read and understood all rules and policies relating to discipline, conduct, and safe use of equipment. I accept responsibility for my/ my child’s behaviour and agree that I/ they will adhere to all guidelines. I understand SFL’s zero-tolerance policy towards misconduct, including bullying, abuse, or unsafe behaviour, and acknowledge that appropriate disciplinary action may be taken. I also accept responsibility for the proper use and care of all equipment and facilities.
6. Cooperation with Training Staff
I agree to cooperate with coaches and academy staff in supporting any necessary modifications to the training program to address my/ my child’s physical, emotional, or behavioural needs (if any).
7. Consent
I grant permission for photographs and/or videos of myself/ my child to be used by Sports For Life (SFL) and its partner academies for official purposes, including digital platforms, social media, and promotional or informational materials.
8. Medical Fitness, Disclosure, and Responsibility
I confirm that I/ my child is medically fit to participate in sports activities and is not suffering from any illness, injury, or condition that may affect safe participation, except as disclosed by me. I have disclosed all relevant medical information, including any illnesses, allergies, medications, or physical conditions, and undertake to promptly inform SFL of any changes. I will ensure appropriate medical advice and care for myself/ my child and exercise due discretion before allowing my child/myself to participate or resume participation after any illness or injury. I further understand and agree that in the event of any undisclosed or pre-existing medical condition, or if any such condition recurs or arises in the present or future, Sports For Life (SFL), its academies, partners, and staff shall not be held responsible or liable for any consequences arising therefrom.
